Case study
Mr. D
Mr. D is a 90-year-old man who was admitted to the hospital with complaints of nausea, vomiting, left arm pain, and chest pain. An electrocardiogram (ECG) is performed, and he is diagnosed as having a myocardial infarction.
Mr. D has a long history of comorbidities including hypertension, diabetes, and congestive heart failure (CHF).
With this in mind, the physician asks Mr. D if he wants life-sustaining measures taken (e.g., CPR, mechanical ventilation, etc.) should he experience cardiopulmonary arrest. Mr. D tells the physician that he wants all measures taken to save his life.
Student post
For Mr. D, a patient with multi-morbidity, receiving life-sustaining measures is beneficial because, in the face of a cardiac arrest, the medical caregivers can help extend his life. Hence, he would not lose his life despite all the risk factors he has that make his mortality rate higher (Forman et al., 2018). The risk of such measures would be worsening his heart condition, especially after CPR, and the prolonging of life is to continue dealing with the dangers of life he faces having comorbidity
it would be essential to consider if Mr. D has had a history of previous Cardiac arrest incidences. Another factor to consider is the intervention that would be most effective in such complications. it is also crucial to consider if the measure will support Mr. D’s aims and preferences and whether the benefits outweigh the risk if a measure is implemented
I would consider that he is elderly and frail due to his multimorbid state and want to talk to him about his decision (Richardson et al.. 2021). Some of the measures he would be opting for could further complicate him and put him in a vegetative state. My opinion would be for him to be explained the situation and given a chance to make a decision again.
